OUR PATIENT PARTICIPATION GROUP IS IN NEED OF NEW MEMBERS TO HELP US TO SHAPE THE SERVICES WE PROVIDE AT THE SURGERY.   OUR PATIENT PARTICIPATION GROUP MEETING HAS BEEN ARRANGED FOR THURSDAY THE 26TH SEPTEMBER AT 11.00AM.  WHILST THIS IS SHORT NOTICE IF ANYONE WOULD BE INTERESTED IN JOINING THE GROUP AND CANNOT ATTEND THIS WEEK - PLEASE EMAIL NNESTCCG.PEACOCKHEALTHCARE@NHS.NET AND WE WILL ENSURE YOU ARE INCLUDED IN ALL FUTURE CORRESPONDENCE AND MEETING INVITATIONS.

Extra evening and weekend appointments available in local area

You can now book routine GP and Nurse appointments in the evening, at weekends and over bank holidays in your local area.

Pre-bookable appointments are available Monday-Friday, 6.30pm-8pm, plus weekends and bank holidays and you can be seen by a GP, Nurse or Healthcare Assistant.

To book your appointment, contact the surgery during normal opening hours. Please be aware that the appointment may take place at another GP Practice in the area.

If you need urgent medical advice when the surgery is closed, you should continue to call NHS 111.

SUPPORT FOR CARERS AT PEACOCK HEALTHCARE

We are continuing to hold our drop in sessions on the third Wednesday of the month 11 a.m. – 12 noon. Please bear in mind as it is a one to one service and it is a first come first serve basis.  If you cannot come to the drop in but would like to have someone to talk to for advice or to signpost you to another service/provider please call the surgery and ask to speak to Leah who is our carers champion.  If we can help with forms filling, direct you to the appropriate social care or just be a listening ear, please contact us.

BLOOD TESTS

Due to the high demand on HCA appointments, all routine blood tests are now to be done at Park House by appointment only  Tel 0115 9617616,  with the exception of Warfarin and Rheumatoid Arthritis monitoring. Please make sure that you have your ICE request form to take with you to this appointment otherwise no bloods can be taken.  Thank You.

PLEASE MAKE SURE THAT YOU HAVE YOUR ICE REQUEST FORM TO TAKE WITH YOU TO YOUR APPOINTMENT OTHERWISE NO BLOODS CAN BE TAKEN

Pre-Registration with the practice

PLEASE BE ADVISED THAT AS PART OF THE THE REGISTRATION PROCESS YOU WILL STILL NEED TO ATTEND THE SURGERY AND ALSO PROVIDE PROOF OF ADDRESS AND ID (ENERGY BILL/COUNCIL TAX BILL-PHOTO-CARD DRIVING LICENCE/PASSPORT) THIS IS THE MINIMUM REQUIREMENT IN ALL CASES.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct

PreRegistrationIf you wish to pre-register, click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.




Alternatively you may print off a GMS1 Registration Form and New Registration Health Check Form, fill it out and bring it in with you on your first visit to the practice.
