*IMPORTANT PATIENT NOTICES*

GP APPOINTMENT? CAN'T MAKE IT? DON'T NEED IT?  CANCEL IT!

If you  are unable to keep your appointment, please make every effort to cancel it well in advance so that it may be offered to someone else. Non-attendance and cancellations at short notice without a valid reason deprives other patients of essential medical services.  Appointments are often wasted as people do not attend.  These appointments could have been used by other patients.

As from the 5th November 2018 we are extending our processing time for prescription requests from 2 working days to 3 working days.  Please bear this in mind when ordering prescriptions from now on. Thank you.

Extra evening and weekend appointments available in local area

You can now book routine GP and Nurse appointments in the evening, at weekends and over bank holidays in your local area.

Pre-bookable appointments are available Monday-Friday, 6.30pm-8pm, plus weekends and bank holidays and you can be seen by a GP, Nurse or Healthcare Assistant.

To book your appointment, contact the surgery during normal opening hours. Please be aware that the appointment may take place at another GP Practice in the area.

If you need urgent medical advice when the surgery is closed, you should continue to call NHS 111.

A new service offered for all carers at Peacock Healthcare

A one to one drop in sessions will be held at Peacock Healthcare every third Wednesday of the month 11 a.m. – 12 noon. The first drop in session will take place on Wednesday 18th April 2018.  Please bear in mind as it is a one to one service and it is a first come first serve basis.

BLOOD TESTS

Due to the high demand on HCA appointments, all routine blood tests are now to be done at Park House by appointment only  Tel 0115 9617616,  with the exception of Warfarin and Rheumatoid Arthritis monitoring. Please make sure that you have your ICE request form to take with you to this appointment otherwise no bloods can be taken.  Thank You.

New Patients 

We are happy to accept your relatives and friends as our patients if they live in our area. Ask them to bring their medical card to the surgery, proof of address & Previous Doctor’s details. You will then be able to complete a Registration Form.  These forms are quite lengthy so please give yourself about 20 minutes to complete them if you are coming into surgery.  Please also answer each question, even if you write in "not applicable", as this will help to speed up the process when we check the forms. 

We will endeavour to comply with any reasonable preference expressed, but your choice of GP cannot be guaranteed

We will also accept patients out of area, but only if you consent to receiving no home visits.  As you would be out of our catchment area it would not be a timely use of our GP resources and therefore not cost effective.  Whilst your health is our primary objective we also have to consider other factors.   You can register but will be asked to sign a consent form.  No other form of care provided by us would be affected by this.  Just remember though, if you became ill at home you would have to access other primary care providers, eg Urgent care/walk in centres or A&E.

Temporary Residents 

If you need medical attention when away from home, you are entitled to see a Doctor anywhere in the UK. We will be happy to see any friends or relatives staying with you if they need medical attention.
